The Youth Care Transition (YCT) program provides two tracks of service for youth with complex needs who need to transition to a more appropriate level of services. The team helps youth move out of emergency rooms and Minnesota Psychiatric Residential Treatment Facilities (PRTFs) by removing barriers and working alongside youth and families to support them during their transition of care. YCT serves youth who are up to 18 years old or who are still in high school.
MN PRTFS and CABHH Referrals
Youth who are currently admitted to Minnesota Psychiatric Residential Treatment Facilities (PRTFs) or the Child and Adolescent Behavioral Health Hospital (CABHH) in Minnesota and are ready to move to a less restrictive setting, we offer Intensive Transition Services. ITS provides intensive in-home support for 90 days for families with youth discharging from either a PRTF or the CABHH. The goal of the program is to provide support during the transition and stabilization period and reduce the risk of return placement. The team will travel to the youth and family to provide on site support and services. Referrals accepted from the PRTF or CABHH where the youth who are discharging to our service area:
Metro Area Counties: Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, Washington
Southern MN Counties: Dodge, Fillmore, Freeborn, Goodhue, Mower, Olmsted, Steele, Waseca
Emergency Room Providers
Youth who are currently boarding in emergency rooms and are challenged with finding suitable services in transitioning back home or to a shelter, crisis, respite, or group home we offer Placement Coordination along with Intensive Transition Services. The goal of the program is to get youth out of emergency rooms and provide support during the transition and stabilization period and reduce the risk of return visits to the emergency room. Referrals accepted from:
Metro Area Emergency Room Provider Counties: Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington Counties.
Outstate Emergency Room Providers: Mayo Clinic in Rochester and Centracare in St. Cloud

Who does YCT accept?
Any youth up to the age of 18 years old or still in high school who are receiving services from a PRTF, CABHH, or metro area emergency room can be referred for services. Criteria for services include:
- Provider at current facility where youth is residing is recommending youth to transition to a less restrictive setting.
- Agreement from youth, parent and/or guardian to receive services and actively participate in services.
- Identified complex needs or barriers that may prevent youth from successfully transitioning to a less restrictive community-based setting.
How does it work?
Contact the Youth Care Transition team to initiate services. Our team will reach out to you to discuss the case. The team will come onsite to meet with the youth, the care team, and the family. As the youth moves to their new residence, the team will work side-by-side with caregivers to remove barriers, address behaviors, and coordinate services to make a successful transition.
Where are youth placed?
The priority is to reunify youth with their families if possible. The YCT team will engage families early in the discharge process and working side-by-side, equip them with the skills and resources needed to care for the youth. If family is not available, the team will support in locating living options.
How about high-needs youth or those with developmental disabilities?
Yes, the Youth Care Transition program can assist all youth.
How long can we receive services?
Intensive services are provided for up to 90 days after youth transitions to community-based placement setting. The team can assist with finding outpatient services that will continue care once the 90 days with YCT is complete.
How are the services paid for?
All services are paid for via a grant from the Minnesota Department of Human Services. There is no cost to families.
